Medical Conditions

Health Care Plans in OSHC

Children with a diagnosed health condition are required to have a current Medical Management Plan in place in order to attend the service. Any medication provided must be in date, clearly labelled by a pharmacist, and match the details outlined in the child’s medical documentation. These requirements ensure compliance with Regulation 90 of the Education and Care National Regulations.

Three Plans must be completed before bookings can be made

1.Medical Plan/Action Plan (completed by medical practitioner)

2.Risk minimisation plan (completed by family and service)

3.Health Support Plan (completed in service by OSHC staff)

If your child needs medication to be administered during OSHC for a short term minor health condition (e.g. antibiotics) for 1 week or less, please contact service Directors and complete the Medication Agreement.

MEDICAL PLANS must:

  • be completed in full and signed by a medical practitioner
  • Include a start and review date
  • The child’s name, date of birth
  • a photo of the child attached
  • Be provided to the OSHC via enrolment or email

MEDICATION REQUIREMENTS
prescribed, over the counter, complimentary medicines:

  • Medication must be labelled with a pharmacist medication label (including child’s name, instructions for administering medication and dosage, corresponding with Medical Plan).

*Please provide the pharmacy with your child’s Medical Action Plan to ensure the medication supplied matches the details outlined in the plan*

  • Must not be expired.
  • A Medication Agreement must be completed by parent or caregiver if medication is to be administered while attending OSHC. This is not required if listed in an Asthma Care plan, Diabetes Action and Management Plan, or Anaphylaxis/Allergy Action Plan.
  • All medication named on Medication Plans must be provided to the service.
  • If Controlled Schedule 8 medication, oxygen, insulin, pain relief (e.g. paracetamol or Ibuprofen to be administered for longer than 72 hours) is needed to be administered during OSHC, a medical practitioner must complete the Medication Agreement.
  • A medication record must be completed by parent or caregiver and service specifying details for administering their child’s medication during OSHC sessions.

Please contact the service if a Medical Agreement is required.

Linden Park OSHC will send reminders to families via email and Xplor regarding upcoming reviews or expiring Medical Management Plans and medication. Families are responsible for ensuring that updated plans and in-date medication are provided to the service prior to the expiry of existing documentation and medication.
